of promoting the activities of Fort William Baptist Church.

I understand that at the event my child may participate in different activities that are supervised by staff and volunteers. I also agree that my child will be bound by the rules of the leaders of these activities and the rules of any place or organization where the activities may be held. In consideration of my child's participation in this event, I understand that there are certain risks of physical injury or illness associated with these activities and I hereby release, waive, and discharge Fort William Baptist Church including their staff, organizers, volunteers and participants, on my child's behalf from any and all liability, loss, and injury which may be suffered, sustained or arise in any way whatsoever as a result of or in conjunction with these activities.
